Job Summary:
Provides nursing care to patients under the direction of a Registered Nurse, utilizing observational skills to assess nursing interventions and assisting medical staff to ensure optimal patient care within established policies, procedures, and standards.
Essential Responsibilities:
- Upholds Kaiser Permanente's Policies and Procedures, Principles of Responsibilities, and applicable laws.
- Exhibits excellent customer relations to patients, visitors, physicians, and co-workers; shows courtesy, compassion, and respect; conforms to critical service standards.
- Provides therapeutic patient care as per physician and registered nurse directives within scope of practice.
- Transcribes physicians' orders according to policy.
- Communicates with registered nurses to modify care plans; contributes to the development of nursing care plans.
- Administers specified medications via oral, hypodermic, intramuscular, and topical routes.
- Performs treatments such as catheterizations, irrigations, suctioning, throat irrigations, and enemas; provides care for patients in isolation.
- Performs related patient care functions including vital signs, dressing changes, patient education, specimen collection, and culture collection.
- Assists physicians and medical staff during examinations and treatments, providing instrument support, patient support, and assistance as needed.
- Evaluates patient condition, identifies significant findings, initiates emergency procedures, and reports to the registered nurse.
- Identifies learning barriers and provides basic teaching following RN plans.
- Documents patient signs, symptoms, observations, and treatments accurately.
- Ensures proper functioning of care equipment and reports safety concerns.
- Uses effective communication skills.
- Performs duties in other units as assigned to maintain standards of care and safety.
- Participates in staff meetings and training sessions.
- Assists with ordering and distributing supplies and equipment.
- Accepts verbal and telephone orders from physicians.
- Administers oxygen therapy, suctioning, sputum induction, and documents care.
- Performs other duties as required, including coordinating discharge processes and traveling to patient homes.
- Promotes excellent customer service and ensures proficiency with age-related competencies.
Basic Qualifications:
- One (1) year of recent LVN experience in home health, hospice, or acute care within the last three years.
Education:

License, Certification, Registration:
- Driver's License (California)
- Vocational Nurse License (California)
- Basic Life Support
- I.V. Certification
Additional Requirements:
- Ability to drive/travel for patient visits.
