Job description

Job Summary:

Provides nursing care to patients under the direction of a Registered Nurse, utilizing observational skills to assess nursing interventions and assisting medical staff to ensure optimal patient care within established policies, procedures, and standards.

Essential Responsibilities:
  1. Upholds Kaiser Permanente's Policies and Procedures, Principles of Responsibilities, and applicable laws.
  2. Exhibits excellent customer relations to patients, visitors, physicians, and co-workers; shows courtesy, compassion, and respect; conforms to critical service standards.
  3. Provides therapeutic patient care as per physician and registered nurse directives within scope of practice.
  4. Transcribes physicians' orders according to policy.
  5. Communicates with registered nurses to modify care plans; contributes to the development of nursing care plans.
  6. Administers specified medications via oral, hypodermic, intramuscular, and topical routes.
  7. Performs treatments such as catheterizations, irrigations, suctioning, throat irrigations, and enemas; provides care for patients in isolation.
  8. Performs related patient care functions including vital signs, dressing changes, patient education, specimen collection, and culture collection.
  9. Assists physicians and medical staff during examinations and treatments, providing instrument support, patient support, and assistance as needed.
  10. Evaluates patient condition, identifies significant findings, initiates emergency procedures, and reports to the registered nurse.
  11. Identifies learning barriers and provides basic teaching following RN plans.
  12. Documents patient signs, symptoms, observations, and treatments accurately.
  13. Ensures proper functioning of care equipment and reports safety concerns.
  14. Uses effective communication skills.
  15. Performs duties in other units as assigned to maintain standards of care and safety.
  16. Participates in staff meetings and training sessions.
  17. Assists with ordering and distributing supplies and equipment.
  18. Accepts verbal and telephone orders from physicians.
  19. Administers oxygen therapy, suctioning, sputum induction, and documents care.
  20. Performs other duties as required, including coordinating discharge processes and traveling to patient homes.
  21. Promotes excellent customer service and ensures proficiency with age-related competencies.
Basic Qualifications:
  • One (1) year of recent LVN experience in home health, hospice, or acute care within the last three years.
Education:

License, Certification, Registration:
  • Driver's License (California)
  • Vocational Nurse License (California)
  • Basic Life Support
  • I.V. Certification
Additional Requirements:
  • Ability to drive/travel for patient visits.
