Job summary

Adaptive Home Health in Texas seeks an elite Licensed Practical Nurse (LVN) for a full-time, field-based role across patient homes. You will perform skilled nursing tasks, wound care, and patient education while documenting care per physician orders.

This position requires a Texas LVN license, CPR, and at least 3 years of clinical nursing experience, plus a valid driver’s license; travel within the region is expected. Competitive pay and comprehensive benefits offered.

Qualifications

• Current Texas LPN license.
• Three years of clinical nursing experience.
• CPR Certification required.

Responsibilities

• Provide comprehensive home-based nursing care.
• Wound care and infusions as prescribed.
• Administer medications and treatments per physician orders.
• Maintain timely documentation and care plans.
• Coordinate care with physicians, caregivers and facilities.
• Travel to patient homes; serve ~6 patients per day, Mon-Fri.
