Job summary

Adaptive Home Health, LLC in Texas is seeking a Licensed Vocational Nurse (LVN) to join our field-based team. You will deliver skilled nursing visits in patients' homes, under RN/clinical supervision, and coordinate care to keep everyone on track.

This full-time role offers a $70,000 yearly compensation, a productivity-based pay model, and scheduling flexibility. Candidates must hold an unrestricted Texas LVN license, have a valid driver's license, and be willing to travel across the territory.

Qualifications

• Current Texas LVN license (unrestricted).
• Comfort in field-based workflow with escalation judgment.
• Strong communication and documentation skills.
• Reliable follow-through on orders and visit cadence.
• Current Texas driver's license and ability to travel across territory.

Responsibilities

• Deliver skilled nursing visits in the home within LVN scope under RN/clinical supervision.
• Perform focused assessments and escalate promptly to the RN/MD/clinical team with objective information.
• Support medication reconciliation and safety education (within scope).
• Provide patient and caregiver education (teach-back) aligned to the plan of care.
• Support wound care and other skilled tasks per orders and competency.
• Coordinate care with office and interdisciplinary team to ensure timely visits and order adherence.
