Job summary

Heritage at Brandon Place is seeking a Licensed Practical Nurse (LPN) to provide direct nursing care in a skilled nursing setting under the supervision of an RN or Director of Nursing.

You will administer medications, monitor resident health, assist with care plans, and ensure high-quality care in accordance with facility policies and state regulations. Excellent communication and teamwork are essential to coordinate with CNAs and the interdisciplinary team.

Qualifications

• Graduate of an accredited practical nursing program.
• Current LPN/LVN license in state of employment.
• CPR certification required.

Responsibilities

• Provide direct nursing care following care plans and physician orders.
• Administer medications and treatments as prescribed.
• Monitor vital signs and resident responses to treatment; report changes to RN/physician.
• Assist with development and implementation of individualized care plans.
• Supervise and support CNAs; maintain accurate medical documentation.
• Maintain infection control and safety standards; participate in shift reports and team meetings.
