PIH Health is seeking an LVN Discharge Planner to collaborate with physicians, nurses, social workers and patients in Los Angeles to determine the appropriate post-acute level of care and to guide families through transitions.

The role involves providing decision support, managing expectations, ensuring smooth progression through hospitalization, aiding utilization of services, and educating healthcare staff about discharge options and reimbursement pathways.

Qualifications

• Active California LVN license required.
• BLS from American Heart Association.
• Minimum of 2 years’ acute hospital experience.
• Knowledge of third-party payer reimbursement methodologies.
• Strong organizational and prioritization abilities.

The LVN Discharge Planner provides discharge planning by collaboratively determining level of care needs beyond acute care, providing decision support to patients and families and physicians, managing patient and family expectations and ensuring a smooth transition to the next level of care and services. Assure that patients and families proceed efficiently throughout the course of hospitalization. The LVN Discharge Planner assists in the appropriate utilization of hospital services, maximizing reimbursement from all third-party payers, and education of medical/hospital staff about healthcare options.

Required Experience

Required:

• Maintain an active California LVN license.
• LA City Fire Card within 6 months of employment (PHGSH only)
• BLS from American Heart Association
• Minimum of 2 years’ acute hospital experience.
• Computer proficiency

Preferred:

• Understands treatment plans and healthcare delivery system.
