Job summary

Syra Health in Denton, TX is seeking a Licensed Vocational Nurse (LVN) to provide direct nursing care, administer medications, educate patients, and document care in accordance with facility procedures and HHSC requirements.

The LVN will collaborate with the interdisciplinary team, maintain accurate records, and participate in required training. Shifts alternate, weekend availability required, and pre-employment requirements must be completed.

Qualifications

• Must possess a current and valid LVN license.
• Must have at least two years of nursing experience.
• Must provide a current NURSYS report showing all nursing licenses held.
• Must provide two complete professional references from supervisors from two different places of employment.
• Must complete and submit the required skills checklist.
• Must comply with HHSC immunization requirements.
• Must successfully complete the HHSC-required drug screening.
• Must successfully complete HHSC-required fingerprinting.
• Must complete all facility and HHSC-required training prior to or as required following placement.
• Must meet all facility pre-employment and credentialing requirements.

Responsibilities

• Provide direct nursing care to patients in accordance with established care plans and facility procedures.
• Administer medications and treatments as ordered and within the LVN scope of practice.
• Provide medication education to patients and reinforce appropriate medication management practices.
• Complete accurate and timely documentation of medication administration, treatments, patient care, and other required clinical information.
• Monitor patients and report changes in condition or concerns to the appropriate clinical staff.
• Follow facility SOPs and established nursing protocols.
