Job summary

Lone Tree Post Acute in California is actively seeking compassionate Licensed Vocational Nurses (LVNs) to join our nursing team on a full-time basis. The LVN will provide direct nursing care to residents according to care plans and physician orders, and supervise CNAs during shifts.

The ideal candidate holds a California LVN license in good standing, has strong clinical and communication skills, and can work independently or as part of a team.

Qualifications

• Current California LVN license in good standing.
• Skilled nursing or post-acute experience preferred.
• Strong clinical and communication skills.

Responsibilities

• Provide direct nursing care to residents in accordance with care plans and physician orders.
• Supervise and support CNAs during assigned shifts.
• Monitor residents' conditions and report changes appropriately.
• Administer medications and treatments as prescribed.
• Ensure accurate and timely documentation of resident care.
• Maintain a safe, clean, and comfortable environment for residents.
• Follow infection control and safety procedures at all times.
• Communicate effectively with residents, families, physicians, and interdisciplinary team members.
• Assist in admissions, transfers, and discharges as needed.
• Respond promptly to resident needs and emergencies.
