Job summary

Elara Caring in Kokomo, IN is seeking a Licensed Vocational / Practical Nurse to join our home health team delivering compassionate, patient‑centered care across patient homes. You will follow individualized care plans and physician orders, monitor patient progress, and document medications.

You’ll coordinate with the RN Case Manager and care team to update plans as needs change, while communicating with physicians, patients, and families to ensure coordinated, high‑quality care.

Qualifications

• Current State License as an LVN/LPN.
• Graduate of an accredited nursing school (Certificate/Diploma/Associate).
• 1 year of clinical care experience preferred.
• Ability to lift up to 50 pounds and assist with patient transfers.
• Valid driver’s license or reliable transportation for travel between patient homes.

Responsibilities

• Follow each patient's care plan and physician orders.
• Monitor patient progress and report changes to the care team.
• Document medications and update profiles per guidelines.
• Coordinate with RN Case Manager to adjust care plans as needed.
• Communicate with physicians, patients, and families to ensure coordinated care.
• Help prevent avoidable hospitalizations through proactive patient care.
