Job summary

Qualifications

• Graduation from an accredited LVN program and CA LVN license.
• Minimum two years of ambulatory care experience preferred.
• Current CPR/BLS certification preferred.

Responsibilities

• Perform LVN duties within scope of practice and assist providers.
• Coordinate care with care team to meet patient goals.
• Educate patients and manage pre-visit checklists.
