Elara Caring is seeking a Licensed Vocational Nurse Clinical Coordinator to join our team in Texas. The role supports patients, caregivers, and field teams from an office setting, coordinating care to ensure timely, high-quality service.

This position requires a current LVN license, 1+ year of nursing experience in home health, strong organizational skills, and the ability to manage multiple priorities in a fast-paced environment. Bilingual Spanish/English is a plus.

Responsibilities

• Reviewing and processing clinical workflows and notes.

• Assisting with follow-up appointments, health summaries, and pre-authorizations.
• Completing chart audits and ensuring HIPAA compliance.
• Supporting branch with occasional field visits and direct patient care as needed.
• Communicating effectively with patients, families, and clinical teams.

Licensed Vocational Nurse Clinical Coordinator

What You’ll Do:

• Reviewing and clearing Homecare Homebase (HCHB) coordination notes
• Processing administrative and clinical workflows accurately and on time
• Obtaining verbal, written, and faxed physician orders and entering them for review and approval
• Reviewing, updating, and maintaining the At-Risk Registry
• Assisting with patient follow-up appointments, health summaries, pre-authorizations, and patient questions
• Completing chart audits according to Elara Caring policies
• Verifying receipt of Medicare Notice of Non-Coverage documentation
• Protecting patient and employee privacy in accordance with HIPAA requirements
• Supporting the branch with occasional field visits and direct patient care, as needed
• Completing additional clinical or administrative projects as assigned
• Bilingual Spanish and English skills are a plus!

What You’ll Need

• Graduation from an accredited school of nursing
• Current, unrestricted LVN license valid in the state of employment
• At least one year of nursing experience in a home health setting
• Strong organization, communication, and attention to detail
• Ability to manage multiple priorities in a fast-paced environment
