Qualifications

• Completed LVN program from an accredited school.
• Active Texas LVN license in good standing.
• Experience in a skilled nursing facility preferred.

Responsibilities

• Observe, assess, document, and report resident condition during rounds.
• Assure implementation of care plans as directed.
• Administer medications as prescribed.
• Conduct rounds with physician and record visits in clinical records.
• Perform narcotic reconciliation with incoming and outgoing shifts.
• Identify changes and take action per facility policies and procedures.
• Oversee and monitor aides.
• Initiate and complete Incident and Accident Reports per facility policy.
• Discharge residents per facility policy.
• Complete Nursing Assessments prior to physician calls.
• Communicate with resident's contact about status or changes in condition.
• Participate in other special projects as assigned by leadership.
