Job summary

Regency Integrated Health Services is seeking an LVN Charge Nurse at Southpark Meadows Nursing and Rehab Center in Austin, Texas. The PRN position supports quality resident care, medication administration, and coordination with physicians and families.

Must have completed an LVN program and hold an active Texas LVN license. Prior skilled nursing experience is preferred, with ability to perform rounds, documentation, and care plan implementation as needed.

Qualifications

• Has completed an LVN program at an accredited vocational school or college.
• Active Licensed Vocational Nurse license in Texas in good standing.

Responsibilities

• Observe, assess, document resident condition during rounds and report changes.
• Ensure implementation of care plans as directed.
• Administer medications as prescribed.
• Record physician visits in clinical records.
• Perform narcotic reconciliation with shifts.
• Identify significant changes and take action per facility policies.
• Oversee and monitor aides.
• Initiate and complete incident/accident reports per policy.
• Discharge residents per policy.
• Complete Nursing Assessment before calling physician on changes in resident condition.
• Communicate with resident's point of contact about status or changes.
• Support special projects or duties as assigned by Director of Nurses or Administrator.
