Job summary

Regency Integrated Health Services in Memorial City Nursing and Rehabilitation Center, Houston, TX is seeking an LVN Charge Nurse to support high-quality resident care. You will participate in rounds, assess residents, and coordinate with physicians to implement individualized care plans.

The role requires current Texas LVN licensure and previous experience in a skilled nursing facility, with full-time, on-site responsibilities and strong teamwork with the nursing staff.

Qualifications

• Has completed an LVN program at an accredited vocational school or college.

Responsibilities

• Observe, assess, document, and report resident condition during rounds.
• Ensure care plans are implemented as directed.
• Administer medications as prescribed.
• Conduct rounds with physicians and record visits.
• Perform narcotic reconciliation with shifts.
• Identify changes in resident condition and act per facility policy.
• Oversee and monitor aides.
• Initiate and complete incident/accident reports.
• Discharge residents per facility policy.
• Complete Nursing Assessment for Physician Calls before contacting the physician.
• Communicate status updates to resident's contact when changes occur.
• Assist with other projects or functions as assigned by DON or Administrator.
