Job summary

Qualifications

• Current LVN licensure in state of practice.
• Current driver’s license and vehicle insurance or access to dependable transportation.
• CPR certification preferred or ability to obtain within 90 days of hire.

Responsibilities

• Provides high quality clinical services within scope of practice as defined by state laws and plan of care.
• Documents patient visits thoroughly in the EMR per regulatory and payer requirements.
• Participates in care coordination, communicates with patient, family, physician and team.
• Function in diverse home settings and with diverse patient populations.
