Responsibilities
- Ensure overall compliance with state and federal laws and regulations in the provision of care and services, in accordance with facility policies, procedures, and professional standards of care.
- Oversee Medication Management Program and supervision of Medication Aides.
- Observe residents for changes in physical, mental, emotional and social functioning.
Essential Job Functions
- Responsible for medication management program, ensuring compliance with state regulations. Ensure appropriate and accurate processing of all physician orders.
- Monitor medication room to ensure proper receiving, ordering, storage, security and disposal of medications.
- Supervision of Medication Aides.
- Completion of Medication Training with ability to train Medication Aides.
- Review 24-hour report daily and ensure appropriate documentation is complete.
- Attend change of shift daily.
- Conduct weekly skin rounds to evaluate progress and effectiveness of current treatments. Provide recommendations as indicated. Report to administrator weekly.
- Complete any treatments which require a licensed nurse.
- Assess residents for need of blood glucose monitoring, intake and output.
- Complete assessment on all new residents within seven days of admission.
- Review pre-placement appraisal, physician's report and other information; converse with care staff and conduct interview with the resident.
- Complete fall, smoking and/or wandering assessments, as indicated.
- Complete resident assessments, identify care needs and initiate/update care and service plans as indicated.
- Provide care plan to administrator for review, signature and completion of any needed negotiated risk agreements.
- Conduct pre-admission appraisals as requested.
- Assess all residents every six months, or as required for changes of conditions or special needs.
- Upon completion of annual assessments, provide list of conferences to be scheduled to receptionist. Coordinate and conduct care conference meetings.
- Review, log, and report monthly weights and vital signs.
- Verify proper transcription of medication orders. Assist with medication distribution.
- Makes rounds of the building at least once per day to identify resident needs and care gaps.
- Maintain knowledge of and comply with applicable federal, state and local laws, regulations, facility policies, procedures and licensing requirements, with ongoing monitoring and review.
- Conduct Department of Social Services Self-Assessment audits and make recommendations for continuous quality improvement.
- Monitor completion of required documentation of care and services, including assessments, physician orders, observations, interventions and response.
- Utilize 24-hour report forms for continuing care and attend change of shift report.
- Complete accident/incident forms and additional reports as required.
- Promote and maintain good public relations.
- Interact effectively, provide leadership, and act as a role model for residents and facility staff.
- Maintain current knowledge of resident status.
- Work with Administrator to ensure all care is provided according to physician orders.
- Direct, practice, and implement infection control programs.
- Ensure employees follow facility procedures for hand-washing, standard precautions and PPE to prevent infection and promote environmental health.
- Ensure proper disposal of sharps.
- Assist staff and problem-solve resident care issues.
- Inform resident, physician, administrator and family or responsible party when incidents occur, there is a significant change in status, or any other pertinent information.
- Demonstrate knowledge of and respect for residents' rights and dignity in all interactions.
- Maintain confidentiality of resident and facility information.
- Promote the highest possible quality of life and level of independence for every resident.
- Act as a resident advocate when necessary and as a mandated reporter of abuse.
- Report relevant information to administrator including compliance issues, quality improvement, resident risk factors, change of condition, medication errors, incidents, refusals of treatment, or other pertinent events.
- Know emergency and disaster plans; remain calm and direct others in emergencies; provide first aid as necessary.
- Perform other job duties as required by manager/supervisor.
Qualifications
- High School Diploma or equivalent required.
- Current, active CA LVN license.
- One-year experience in long-term care preferred.
- Experience providing direct care to the elderly preferred.
- Pass medication dosage and calculation test with a score of 80% or higher.
- Maintain current First Aid certification.
- Maintain current CPR or BLS certification.
- Ability to obtain and maintain California Background Clearance through the Department of Social Services.
- Good physical and mental health.
- Well-groomed professional appearance.
- Effective interpersonal and communication skills.
- Strong organizational and leadership skills; good judgment and problem-solving.
- Ability to understand and follow written and/or verbal directions; prioritize within established time frames.
- Flexible schedule including nights, weekends and holidays; ability to work independently.
- Strong attention to detail and excellent customer service.
- Proficient in Microsoft Office is a plus.
Language
- Must be able to fluently speak, read and write English.
- Fluent in Chinese (Cantonese and/or Mandarin) preferred.
- Fluency in other languages is an asset.
Status
This is a Fair Labor Standards Act Non-exempt position.
