Fair Oaks is seeking a Licensed Vocational Nurse to provide direct medical and psychiatric nursing care to residents, including assessments, care plan development, medication administration, and physician-directed treatments. The LVN collaborates with physicians, nursing leadership, case managers, and interdisciplinary staff to ensure safe, effective care.
Candidates must hold an active California LVN license, BLS certification, and be able to pass background and health screenings.
The Licensed Vocational Nurse provides direct general medical and psychiatric nursing care to residents, including completion of all facility-designated assessment instruments assigned to the LVN, collection and documentation of assessment data, development, review, and updating of resident care plans within the facility's level of care, implementation of physician orders and care or treatment plans, medication administration, prescribed treatments, monitoring, documentation, and timely communication of changes in condition. Assessment data and care plans requiring Registered Nurse review are submitted for RN clinical review, analysis, concurrence or direction for revision, and signature in accordance with facility policy and applicable requirements.
All medical and psychiatric care, treatment decisions, and medication regimens are directed by the treating physicians. The LVN practices under the direction of a licensed physician or Registered Nurse. The Director of Nursing provides nursing oversight, competency validation, clinical guidance, and monitoring of nursing practice. The Administrator provides administrative and operational supervision. The LVN works collaboratively with physicians, the Director of Nursing, the Administrator, pharmacy personnel, case managers, therapists, support staff, Mental Health Workers, Activities staff, and other members of the interdisciplinary team.
The LVN may be assigned duties commonly performed by a Certified Nurse Assistant, support staff member, Mental Health Worker, or Activities staff member when trained and competent. The employee remains accountable as an LVN and must continue to meet all assigned licensed nursing responsibilities during the assignment. Such assignments must not delay or interfere with safe medication administration, treatments, monitoring, documentation, change-of-condition response, or other required licensed nursing functions.
Shift: 5 P.M. - 5:30 A.M., NOC
