Job summary

Forest Park Nursing & Rehabilitation in Dallas, TX is seeking a Licensed Vocational Nurse (LVN) to provide outstanding patient care in accordance with standards and orders. The role will focus on accurate documentation, adherence to policies, and collaboration with the care team to meet resident needs.

Responsibilities include charting, narcotic control, and coordinating patient care with physicians. A commitment to compassionate, compliant service and state guidelines is essential for success

Qualifications

• Current LVN/LPN license in practicing state.
• Current BLS/CPR certification or willingness to acquire one.
• Experience in nursing management is preferred but not required.

Responsibilities

• Follow established procedures for charting and incident reporting.
• Follow procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Document changes in a patient's condition including nutrition, infections, skin conditions, weight loss or other changes.
• Utilize the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.
• Make rounds with physicians when necessary.
