Montefiore Medical Center in Bronx, NY is seeking an LVAD Coordinator within the Department of Cardiothoracic Surgery. The role involves planning, evaluation, coordination, implementation and facilitation of care for patients requiring mechanical circulatory support and heart transplantation.

You will educate VAD patients and families, manage inpatient/outpatient care, ensure protocol adherence, and participate in remote on-call coverage with an LVAD hotline.

Qualifications

• Master's degree in Nursing is required.
• Current New York State license and registration as a Registered Professional Nurse (RN) is required.
• Current New York State Nurse Practitioner certification and registration OR NCCPA certification is required.
• Three (3) years of clinical experience in a highly specialized, complex care program preferred.

Responsabilités

• Coordinates and provides education to ventricular assist device (VAD) patients and their families.
• Coordinates the comprehensive inpatient and outpatient clinical management of the ventricular assist device population.
• Ensures adherence to established protocols and program certification requirements.
• Participates in remote on-call coverage for approximately four shifts per month, providing emergency transplant/LVAD hotline management.
