Job summary

Qualifications

• RN license required and valid.
• One year RN experience preferred.
• Experience with LVAD/Heart failure care preferred.

Responsibilities

• Provide face-to-face and non-face to face patient care in CV Surgery/LVAD outpatient setting.
• Coordinate nursing care, educate patients and families, and ensure transitions of care.
• Collaborate with providers and care team; participate in quality improvement efforts.
