Alumus seeks a Certified Nursing Assistant to provide personal care to residents in their homes under the supervision of a registered nurse. You will assist with admissions, transfers, and discharges and ensure care plans are followed to meet individual needs.

Responsibilities include reviewing care plans daily and adapting to changes in daily routines, with emphasis on compassionate, patient-centered support in a home setting.

Qualifications

• Arizona CNA license certification prior to hiring.
• Must have a current CPR and fingerprint clearance card.
• Minimum of six (6) months CNA/LNA experience preferred.
• Recent Long-Term Care/ Skilled Nursing Facility or Transitional Rehab experience
• LTC/SNF/Rehab preferred.

Responsabilités

• Assist guests/patients during admissions, transfers, and discharges. Check each guest/patients routinely to ensure that his/her personal care needs are being met in accordance with his/her wishes and the care plan.
• Review care plans daily to determine if changes in the guests/patient's daily care routine have been made on the care plan.
