Job summary

The U.S. Department of Veterans Affairs is seeking a Registered Nurse - Lung Cancer Screening Clinical Coordinator in Charleston, SC.

The role provides and/or supervises direct patient care, coordinating care across providers and settings, with a focus on education, self-management, and patient satisfaction. Responsibilities include chart reviews for eligibility, shared decision-making with enrollees, and leading in-services to educate staff on the Lung Cancer Screening Program.

Qualifications

• Responsible for all elements of the nursing process while providing direct patient care.
• Assesses, plans, implements, and evaluates age-specific care components.
• Coordinate care focused on education, self-management and satisfaction.
• Provide patient management, care coordination and disposition planning.
• Assist Veterans, families, and caregivers with options and services.
• Coordinate care with multiple providers across care levels and sites.
• Manage consults for lung cancer screening enrollment by chart reviews.
• Conduct shared decision-making with potential enrollees.
• Lead in-services to educate staff on the Lung Cancer Screening Program.
