LTSS Service Coordinator-LPN Case Manager (St. Petersburg/Tampa)

Join to apply for the LTSS Service Coordinator-LPN Case Manager (St. Petersburg/Tampa) role at Elevance Health.

This is a field-based role located in Hardee, Highlands, Hillsborough, Manatee, Pasco, Pinellas, or Polk counties, requiring travel to client sites or designated locations, with occasional office attendance for meetings or training. The schedule is Monday-Friday, 8am-5pm EST.

Responsibilities include:
  1. Assisting the responsible RN in identifying high-risk members and obtaining clinical data.
  2. Supporting care coordination for members with chronic illnesses, co-morbidities, and disabilities.
  3. Participating in developing and revising care plans to meet member needs.
  4. Contributing to the efficient utilization of health benefits.
Minimum Requirements:
  • Licensed LPN/LVN, LSW, LCSW, LMSW, or other applicable license in accordance with state law, with at least 2 years of experience working with individuals with chronic conditions in a Service Coordinator or similar role.
  • Current, unrestricted license in the applicable state.
Preferred Skills:
  • Bilingual in Spanish.
  • Previous case management or oncology experience.
  • Proficiency in Microsoft Office Suite.
