Job summary

Jobtailor in Roanoke, VA is seeking an experienced licensed practical nurse or social work/licensed social worker to join a care coordination team. The role works under the supervision of an RN to support members with chronic illnesses and disabilities.

You will gather clinical data, identify high-risk members, coordinate care plans, and assist in developing, monitoring, and revising plans in collaboration with RNs and care teams.

Qualifications

• Requires an LPN/LVN, LSW, LCSW, or LMSW
• Nursing Diploma or AS in Nursing or a related field
• minimum of 2 years of experience with chronic illnesses, co-morbidities, and/or disabilities
• Current, unrestricted LPN/LVN, LSW, LCSW, LMSW or license other than RN in applicable state(s) required
• May require state-specified certification based on state law and/or contract

• Obtain clinical data as directed by the RN
