Job summary

Jobtailor is seeking a Service Coordinator to oversee and adapt individualized service plans for members with chronic illnesses and disabilities in Orlando. You will coordinate care with caregivers, family, and physicians while addressing non-clinical needs and advancing care goals.

The role emphasizes collaborative mentorship and effective communication, with bilingual Spanish/English preferred to support diverse member needs and improve service delivery.

Qualifications

• BA/BS degree required or equivalent.
• Minimum 2 years of experience with a social work agency.
• Minimum 5 years of experience with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator or similar role.

• Develops, monitors, and revises the member's service plan to meet the member's needs.
• Perform telephonic or face‑to‑face history and program needs assessments.
• Identify members with potential clinical health care needs and coordinate those member's cases.
• Manage non‑clinical needs of members with chronic illnesses, co‑morbidities, and/or disabilities.
• Establish short and long‑term service and support goals in collaboration with the member, caregivers, family, natural supports, physicians.
• May also serve as a mentor or preceptor for new staff.

Requirements

• BA/BS degree.
• Minimum 2 years of experience working with a social work agency.
• Minimum 5 years of experience in working with individuals with chronic illnesses, co‑morbidities, and/or disabilities in a Service Coordinator or similar role.
• Bilingual in Spanish/English preferred.
• Specific education and years and type of experience may be required based upon state law and contract requirements.
