Location: This field-based role enables associates to primarily operate in the field, traveling to client sites or designated locations as their role requires, with occasional office attendance for meetings or training. This approach ensures flexibility, responsiveness to client needs, and direct, hands-on engagement.

The MyCare Ohio health plan aims to deliver high-quality, trauma-informed, culturally competent, person-centered coordination for all members, addressing physical health, behavioral health, long-term services and supports, and psychosocial needs.

The LTSS Service Coordinator-Clinician is responsible for working under the supervision of an RN, with overall responsibility for the member's case. As required by applicable law and contract, the Clinician contributes to the LTSS care coordination process by performing activities within the scope of licensure, such as assisting the RN with assessments for the identification, evaluation, and management of member's needs, including physical health, behavioral health, social services, and long-term supports.

How You Will Make An Impact

• Assist the RN in identifying high-risk members and obtain clinical data as directed.
• Identify members who may benefit from alternative care levels or waiver programs.
• Provide collected information to the RN for verification, further assessment, and care plan development.
• Coordinate care for members with chronic illnesses or disabilities, ensuring cost-effective utilization of health benefits.
• Make decisions based on current member needs, understanding disease processes and applying clinical guidelines, without requiring nursing judgment.

Minimum Requirements

• Licensed practical nurse (LPN/LVN), social worker (LSW, LCSW, LMSW), or other applicable license, with a diploma or associate degree in nursing or related field, and at least 2 years of experience working with individuals with chronic conditions or disabilities in a similar role, or equivalent education and experience.
• Current, unrestricted license in applicable state(s).

Preferred Skills and Experience

• Case management experience with older adults or individuals with disabilities preferred.
• BA/BS in health or nursing is preferred.
