Job description

LTSS Service Coordinator - Clinician

Location: This is a field-based position. Candidate should reside in Nueces County, TX.

The LTSS Service Coordinator-Clinician is responsible for working under the supervision of an RN, with overall responsibility for the member's case. As required by applicable state law and contract, the clinician contributes to the LTSS care coordination process by performing activities within the scope of licensure, including assisting the RN with assessments for identifying, evaluating, and managing members' needs across physical health, behavioral health, social services, and long-term services and supports.

How you will make an impact:

  • Assist the RN in identifying high-risk members and obtaining clinical data as directed.
  • Identify members who may benefit from alternative care levels or waiver programs.
  • Provide collected information to the RN for verification, interpretation, and further assessment, aiding in developing and updating the member's care plan.
  • Coordinate care for members with chronic conditions or disabilities, ensuring cost-effective utilization of health benefits.
  • Make decisions based on current member needs, understanding disease processes and clinical guidelines, without requiring nursing judgment.

Minimum Requirements:

  • Hold an LPN/LVN, LSW, LCSW, LMSW, or other applicable license, with a nursing diploma or associate degree, and at least 2 years of experience working with individuals with chronic illnesses, co-morbidities, or disabilities in a service coordination or similar role, or equivalent education and experience.
  • Current, unrestricted license as required by the state law.
  • May require state-specific certification based on law or contract.

Preferred Skills, Capabilities, and Experiences:

  • MA/MS in Health or Nursing preferred.
  • Willing to travel to work sites and other locations as necessary.
