Responsibilities

• Managing service coordination for a designated caseload in specialized programs.
• Collaborating with clients to lead Person Centered Planning process, documenting preferences, needs, and goals.
• Conducting assessments and creating comprehensive Person Centered Support Plans (PCSP).
• Participating in interdisciplinary care rounds to establish integrated care plans.
• Engaging client support networks and overseeing management of physical, behavioral, and long‑term services.
• Performing face‑to‑face program assessments, utilizing motivational interviewing techniques.
• Coordinating waiver needs and serving as single point of contact with healthcare teams.
• Managing non‑clinical needs of members with chronic illnesses or disabilities for efficient utilization.
• Documenting short and long‑term service goals collaboratively with client and care team.
• Identifying alternative service levels or other waiver programs for clients.
• Mentoring, training, and contributing to process improvement initiatives.
• Submitting utilization/authorization requests and supporting documentation.
• Reporting critical incidents to internal and external parties such as state and county agencies.
• Assisting with appeals, grievances, and state audits.

Location: Columbus, OH. This field‑based role requires traveling to client sites with occasional office attendance.

Responsibilities

• Managing service coordination for a designated caseload in specialized programs.

Minimum Requirements

• BA/BS degree and minimum of 2 years of experience working with a social work agency, or equivalent combination of education and experience.

Preferred Skills and Experience

• Strong preference for case management experience with older adults or individuals with disabilities.
• BA/BS in Health or Nursing preferred.

Compensation and Benefits

Salary range: $26.19 per hour to $32.75 per hour. Elevance Health offers a comprehensive benefits package and additional incentives.
