A leading healthcare company seeks an LTSS Service Care Manager for its Naples, Florida location. This role involves developing and coordinating care plans for long-term care members while ensuring quality healthcare outcomes. The position requires extensive field travel, emphasizing independence and flexibility in scheduling, with an emphasis on managing high caseloads in a home health or geriatric setting.
Position Title: LTSS Service Care Manager
Work Location: Collier - Naples, Florida (Region 8) (requires extensive field travel, 80% of time)
Assignment Duration: 3 months with intent to convert
Work Schedule: Monday to Friday, 8 AM - 5 PM
Work Arrangement: Remote with 80% field visits required
Position Summary:
Assists in developing, assessing, and coordinating holistic care management activities to enable quality, cost-effective healthcare outcomes.
May develop or assist with developing personalized service care plans/service plans for long-term care members and educates members and their families/caregivers on services and benefits available to meet member needs.
Background & Context:
Team has strong longevity; many have been with the team for years.
Remote + fieldwork = highly independent role.
Flexible scheduling; field nurses can plan their weeks.
Monthly and quarterly member contact requirements; 80% travel; must have driver's license.
Goal: Case management for geriatric LTC members.
Key Responsibilities:
Evaluates member needs, available resources, and facilitates best outcome plans.
Assists in developing LTC service plans and identifies needed providers or resources.
Coordinates between member/family/caregivers and care team to ensure service access.
Provides support for employment, housing, justice, foster care, etc.
Monitors and updates service plans based on member needs and outcomes.
Engages with healthcare providers to ensure needs are met.
Maintains member data and care activities per regulations.
May perform home/site visits to assess member needs.
Educates members/families on procedures, benefits, and options.
Offers feedback to improve care/service quality.
Performs other duties as assigned.
Complies with all policies and standards.
Qualification & Experience:
Requires a Bachelor's degree and 2-4 years of related experience.
OR equivalent experience reflecting role scope and skill level.
License/Certification (state-specific):
Iowa: Bachelor's in human services or RN with 4+ years' experience.
North Carolina (Standard Plan): 2+ years of LTSS/HCBS and care management. RN or LCSW required.
North Carolina (Tailored Plan): Same as above. RN or LCSW/LCSW-A preferred.
Candidate Requirements (Additional):
Bachelor's in healthcare-related field (e.g., Psychology, Sociology).
Valid driver's license required.
Must Have:
2+ years of field-based care management
LTC Medicaid experience
Comfortable managing high caseloads (50-70+)
Tech-savvy, familiar with EMR/EHR systems
Home health or geriatric experience
Nice to Have:
Discharge planning
TruCare software experience
Disqualifiers:
No field experience
No high caseload handling
