Job summary

MyMichigan Health is seeking a dedicated LTC Nurse Aide / PCT to support patient care according to each individual's plan of care. This role involves providing hands-on personal care, assisting with daily living activities, and maintaining infection control standards.

You will document care, report changes to the registered nurse, and work within an interdisciplinary team to promote patient comfort and safety.

Qualifications

• High School Diploma or GED or equivalent experience.
• CENA - Cert Educated Nurse Aide, required within 4 months.
• BLS - Basic Life Support, required within 1 month of hire.
• Fingerprinting required.
• Ability to communicate effectively in English.

Responsibilities

• Provides hands-on personal care per care plan.
• Reports changes in patient condition to RN.
• Completes records per policy.
• Assists with ambulation and activities of daily living.
• Maintains infection prevention practices and safety.
• Works as part of interdisciplinary team.
