Job summary

Apex Home Health, Hospice & Home Care is seeking an Experienced LTAC Registered Nurse to provide skilled care for medically fragile adults in a long-term acute care setting. You will manage ventilator support, wound care, IV therapy, and telemetry, coordinating with an interdisciplinary care team.

You will educate patients and families about care plans and treatment goals while maintaining infection control and adherence to clinical protocols. Strong clinical judgment and teamwork are essential.

Qualifications

• Current RN licensure with LTAC experience preferred.
• Ability to manage high-acuity, medically fragile patients.
• Strong communication and teamwork to coordinate with multiple disciplines.

Responsibilities
