Job summary

SnapCare is seeking a Registered Nurse for a 13-week assignment on a day shift in Kentucky. The RN will conduct initial assessments within 8 hours of admission and develop individualized care plans. Supervision of LPNs/CNAs and direct patient care will be key responsibilities.

The role includes participation in rehabilitation activities, ongoing patient status updates for care conferences, and discharge planning from day one, with thorough documentation and communication with the attending team.

Responsibilities

• Perform nursing assessments on all patients within 8 hours of admission to develop individualized care plans
• Supervise Licensed Practical Nurses, Certified Nursing Assistants, and/or orientees
• Provide direct patient care utilizing the nursing process of assessing, planning, implementing, and evaluating patient care
• Participate in patient rehabilitation program on assigned unit
• Provide patient information and status updates for Initial Team Conferences and weekly Re-evaluation Team Conferences
• Initiate discharge planning from the day of admission and patient teaching in anticipation of discharge date
• Maintain accurate and timely documentation of all assessments, interventions, and patient responses in the medical record
• Communicate pertinent information regarding changes in patient status to attending physician and Charge Nurse
