Job summary

HealthPro Heritage is seeking a Licensed Physical Therapist Assistant (LPTA) to support patient care in senior living facilities in Flossmoor, IL. The role offers per-diem, consistent hours, and opportunities to float to Olympia Fields.

You will assist in implementing treatment plans, document patient progress, and collaborate with a multidisciplinary team to deliver high-quality care aligned with regulatory standards.

Qualifications

• Associate’s degree in Physical Therapist Assisting from an accredited program.
• Valid state licensure as a Physical Therapist Assistant, or license eligible.
• Strong assessment, diagnostic, and therapeutic abilities with excellent communication and interpersonal skills. Proficient in using diagnostic tools, therapy equipment, relevant software, and electronic health record systems, with a proven ability to work collaboratively in a multidisciplinary team.

Responsibilities

• Assist in implementing treatment plans designed by Physical Therapists. Provide direct patient care including therapeutic exercises, manual therapy, and modality applications.
• Monitor and document patient progress, report observations to the Physical Therapist, and educate patients and families on treatment protocols, exercises, and self-care techniques.
• Accurately document patient treatments, progress, and responses in compliance with facility policies and regulatory standards.
• Work closely with Physical Therapists, other healthcare professionals, and support staff to ensure coordinated patient care and effective treatment outcomes.
