Responsibilities

The LPN in Home Health provides quality patient care and performs technical skilled care in patients' homes, within the scope of practice defined by state laws, in accordance with the care plan, and in coordination with the healthcare team. May not alter the care plan.

• Instructs patients' families in patient care and maintaining a healthy environment.
• Observes, records, and reports patient responses and condition changes to the supervising nurse or physician daily.

Education and Experience

License Requirements:

• Current LPN licensure in the state of practice.
• Valid Driver's License, vehicle insurance, and access to a reliable vehicle or public transportation.
• CPR Certification.
