Job summary

Ascension is seeking an LPN/LVN to join the Wound Care team at Ascension Jane Phillips Medical Center in Bartlesville, OK. The role involves wound care delivery, patient prep, and collaboration with physicians to ensure efficient clinic flow.

You will provide patient education on home wound management, document progress, and participate in care planning with the multidisciplinary team. LPN/LVN licensure and BLS are required.

Responsibilities

• Deliver specialized wound care treatments and dressing changes to accelerate healing.
• Manage patient intake, prep sterile treatment rooms, and assist during procedures to optimize clinic flow.
• Educate patients and caregivers on home wound care to reduce readmissions.
• Monitor and communicate changes in wound progression to the care team.
