Job summary

Delta-T Group seeks an LPN for a Richmond-area group home, providing skilled nursing care and medications as prescribed. You will monitor residents, document treatments, and work with a multidisciplinary team to ensure comprehensive care and safety.

Ideal candidates have an active Virginia LPN license, experience with G-tube feedings, and strong communication skills to educate residents and caregivers. Reliable transportation is preferred for commuting between locations.

Qualifications

• LPN license in good standing (Virginia) or multi-state privilege eligibility.
• Experience with medication administration and G tube feeding.
• Knowledge of intellectual and developmental disabilities preferred.
• Strong communication and interpersonal skills.
• Ability to work independently and with a multidisciplinary team.
• Reliable transportation to group home locations.

Responsibilities

• Administer medications and treatments as prescribed by healthcare providers.
• Monitor and assess residents' health status and report changes to the healthcare team.
• Provide skilled nursing care including wound care and medical interventions as needed.
• Perform G tube feedings and ensure proper nutrition and hydration.
• Maintain accurate documentation of medication administration and medical treatments.
• Collaborate with caregivers and healthcare professionals for coordinated care.
• Educate residents and caregivers on health management and medication protocols.
