Job summary
ViaQuest Community Solutions is seeking a Wellness LPN for full-time, in-home care across Indiana. The role focuses on wellness coordination under RN supervision, caring for individuals in their residences, and administering medications as directed.
A strong desire to serve individuals with developmental disabilities is essential. Requirements include an accredited nursing program graduate, Indiana LPN license, background check, auto insurance, and a valid driver's license that meets company
Qualifications
- RN supervision not required for daily tasks, but coordination with RN is expected.
Responsibilities
- Develop, oversee and maintain wellness coordination plans.
- Assess and care for individuals in their residence.
- Administer and instruct administration of medication.
- Assist in employee development and site training.
- Serve as a member of the interdisciplinary team.
- Promote health and safety and self-esteem.
- Assist individuals in learning self-care techniques.
- Support nursing care as directed by the RN.
