Job summary

Interim HealthCare Inc. in Erie, PA is seeking compassionate LPNs/RNs for full-time, in-home patient care. You will be called to care when you're needed most, delivering comfort and dignity to our clients in one-on-one visits.

Join a team that supports start-of-care for new cases, provides daily pay options, and offers a stable schedule with primarily first shift and occasional third shift; overtime is not required.

Qualifications

• LPN or RN license valid in the state.
• Current CPR/AED/BLS/First Aid certification.
• Reliable transportation to/from care sites.
• Home health experience preferred but not required.

Responsibilities

• Provide in-home direct patient care according to care plan and scope of practice.
• Assess and document patient conditions, report changes as needed.
• Administer medications, insulin, and IV fluids per orders.
• Inspect wounds, change dressings, and assist with personal hygiene.
• Collaborate with providers and care team to achieve patient outcomes.

As a LPN/RN you will:

• Providein-homedirect patient care according to theprovider'sprescribed plan of care (and nursingscope of practice) while closely monitoring progress/recovery
• Assessand document patient conditions and reportchangeas needed (vitals,blood/stool/urine, glucose,etc.)
• Administer medication, insulin, and IV/fluids,documenting thoroughly
• Inspect and care for wounds, changing dressings and assisting with personal hygiene
• Assess patient for further injuries, infections, bedsores, etc. and deliver appropriate treatments
• Collaborate with providers, aides, and allied health professionals to ensure exceptional patient outcomes

To qualify for a LPN/RN position with us, you will need:

• RN/LPN License
• Current CPR/AED/BLS/First Aid certification
• Reliable transportation to/from care sites and/or work locations
• Practical trachand/or ventilator experience preferred, not required
