Job summary

Vitality Hospice & Palliative Care is seeking a Per Diem Licensed Practical Nurse (LPN) for weekend shifts. You will provide compassionate hospice nursing in patients’ homes and care facilities, guided by individualized plans of care. This role offers flexibility, autonomy, and meaningful one-on-one patient care.

Responsibilities include ongoing assessments, pain and symptom management, ADL assistance, education for families, and timely documentation. On-call rotation is optional based on needs.

Qualifications

• Active LPN license in the state of practice.
• Minimum 1 year of nursing experience (hospice preferred).
• Strong assessment, communication, and documentation skills.

Responsibilities

• Provide skilled hospice nursing care per patient care plan.
• Perform nursing assessments and report changes promptly.
• Support pain and symptom management for comfort and quality of life.
• Assist patients with ADLs as needed.
• Educate patients, families, and caregivers on care needs and self-care.
• Document visits and care provided accurately and timely.
• Collaborate with interdisciplinary hospice team for coordinated care.
• Participate in on-call rotation as available (optional).

Vitality Hospice & Palliative Care is seeking a dedicated Per Diem Licensed Practical Nurse (LPN) to provide high-quality, patient-centered care to individuals receiving hospice services in their homes or care facilities.

This per diem role offers flexibility, autonomy, and meaningful one-on-one patient care, making it ideal for nurses who want to make an impact while maintaining schedule freedom.

Position Details

• Per Diem / As-Needed
• Flexible scheduling

Key Responsibilities

Under the direction of an RN Case Manager and clinical leadership, the LPN will:

• Provide skilled hospice nursing care in accordance with the patient's individualized plan of care
• Perform nursing assessments and promptly report changes in patient condition
• Support pain and symptom management to promote comfort and quality of life
• Assist patients with activities of daily living (ADLs) as needed
• Educate patients, families, and caregivers on care needs and self-care strategies
• Accurately document visits and care provided in a timely manner
• Collaborate with the interdisciplinary hospice team to ensure coordinated, holistic care
• Participate in on-call rotation as available (optional, based on per diem needs)

Qualifications

• Active and unencumbered LPN license in the state of practice
• Minimum 1 year of nursing experience preferred (hospice experience a plus)
• Knowledge of hospice philosophy and care for terminally ill patients
• Strong assessment, communication, and documentation skills
• Ability to work independently and manage time effectively
• Valid driver's license, reliable transportation, and current auto insurance
