Catholic Health Hospice is seeking a compassionate Licensed Practical Nurse to join our community hospice care team. This per-diem role covers evenings and weekends, providing medical and emotional support to patients and families in a home setting.
The LPN will work under RN supervision, communicate patient status to the care team, and assist with pain and symptom management at end-of-life. NYS LPN license and driver’s license required.

Per Diem requirement is 6 shifts per month, covering evenings and weekends.
