Responsibilities

The Licensed Practical Nurse is responsible for providing direct nursing care as prescribed by the physician to acutely ill and/or chronically ill patients in their homes. This includes recording clinical and progress notes and coordinating other patient care activities/services to ensure high-quality care within the assigned geographic area. The nurse works under the supervision of the Registered Nurse or designated supervisor. All nursing activities are performed in compliance with the State LPN/LVN Practice Act, applicable licensure/certification requirements, and agency policies and procedures.

Specific Job Duties/Responsibilities
  • Provides high-quality clinical services within the scope of practice, as defined by state laws, in accordance with the plan of care, and in coordination with the healthcare team, following all infection control procedures.
  • Documents patient visits thoroughly and completely in the electronic medical record, utilizing the agency’s electronic visit verification program (as applicable).
  • Participates actively in care coordination, including reporting patient needs and changes in condition, attending case conferences, and communicating effectively with patients, families, physicians, and other team members.
  • Ensures confidentiality of patient health information per agency policy.
Education and Experience

Education Requirements

  • Must meet educational and experience requirements outlined in state licensure laws.
  • Minimum of one (1) year of acute care experience preferred; previous home care experience is also preferred.
  • Current LPN licensure in the practicing state is required.
  • Valid driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation are necessary.
  • Current CPR certification required.
