Description

As a Licensed Practical Nurse (LPN) Visit Nurse, you will be called to care when you’re needed most. As part of Interim Healthcare Home Health, you’ll support a full range of patient services to bring comfort and dignity to our clients.

As an LPN Visit Nurse, you will:

• Provide in-home direct patient care according to the prescribed plan of care while monitoring progress.
• Complete skilled assessments and deliver patient-centered care in the home.
• Ensure accurate evaluation and implement a comprehensive care plan addressing patient needs.
• Communicate patient conditions and collaborate with providers to adjust care as needed.
• Document all clinical activities per agency protocol and federal guidelines.
• Educate patients and families involved in the care process.
• Help decrease readmissions by promoting preventative care and ensuring continuity of care.

To qualify as an LPN with us, you need:

• Graduation from a practical (vocational) nursing program and a current LPN license in your practicing state.
• At least 1200 hours of experience as an LPN or completion of the Office's preceptor program.
• Current CPR/AED/BLS/First Aid certification.
• Reliable transportation to care sites.
• One year of experience in home health or a similar setting.
