Job summary

The Veterans Health Administration, Saint Cloud VA Medical Center in Saint Cloud, MN is seeking a Licensed Practical Nurse (LPN) to provide skilled nursing care in a Community Living Center. The role supports short- and long-term resident care, medication administration, vital signs monitoring, and daily living activities in a compassionate, patient-centered environment.

This 24/7 unit offers day, evening, and night shifts with alternating weekends and holidays.

Qualifications

• Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or NLNAC accredited at time of program completion
• Active, full, current, unrestricted licensure as a Graduate LPN/LVN in the United States or DC
• May qualify under grandfathering provisions for current VA employees in this occupation

Responsibilities

• Provide hands-on nursing care including medication administration, vitals, and daily living assistance
• Observe, document, and report changes in patient condition to RN/MD
• Collaborate with RNs, physicians, therapists, and social workers to implement care plans
• Maintain detailed health records and patient education per VA guidelines
• Support patients and families with education and end-of-life care as appropriate
