Job summary

The Plaza Health and Rehabilitation Center in Gainesville, FL seeks a dedicated LPN Unit Manager to oversee daily nursing operations on the assigned unit. You will coordinate resident care, supervise LPNs and CNAs, and ensure compliance with facility policies and regulations to promote safe, resident-centered care and high-quality outcomes.

Qualifications

• Active Florida LPN license or eligibility
• Graduate of an approved practical nursing program
• Minimum 2 years nursing experience in SNF/rehab
• Prior supervisory/charge nurse experience preferred
• Proficient in EMR and nursing documentation

Responsibilities

• Oversee daily nursing operations and coordinate resident care activities within the unit
• Provide leadership, guidance, and support to LPNs and CNAs
• Monitor nursing staff performance and ensure duties are completed accurately
• Assist with staff scheduling, orientation, training, and evaluations
• Promote teamwork and effective communication
• Identify staffing or performance concerns and report to DON/ADON
• Coordinate care per physician orders and care plans
• Monitor residents and communicate changes to providers
• Administer medications and treatments per FL LPN scope
• Participate in assessments, care planning, and conferences
• Ensure timely documentation of care and changes
• Assist with admissions, transfers, discharges, and care coordination
• Monitor infection control, fall prevention, skin integrity, and safety
• Communicate professionally with residents, families, and team
• Ensure compliance with regulations and quality initiatives
• Maintain unit records and supplies
• Investigate incidents and participate in leadership meetings
