Job summary

Roosevelt Care Centers in New Jersey is seeking a Licensed Practical Nurse to provide direct nursing care in a long‑term care facility. In the absence of an RN, you will act as Team Leader in a unit and support care planning under RN direction.

Responsibilities include medication administration, charting, wound care rounds, and collaboration with physicians and families. This is a union position with PTO, pension benefits, and a competitive hourly wage.

Qualifications

• Ability to read, write, speak and understand English sufficiently to perform the job.
• Graduation from an accredited school of practical nursing plus a current NJ license to practice as a Licensed Practical Nurse.

Responsibilities

• Provides direct care, including medication administration and treatments to residents.
• Monitors residents' condition and alerts RN or Nursing Supervisor as needed.
• Documents resident responses and nursing interventions in medical records.
• Monitors medications, lab results and therapies as applicable.
• Assists in completing MDS and care planning under RN direction.
• Orientates residents and families to the long-term care environment and liaises with families.
• Participates in committees and conferences and assists other clinical staff.
• Initiates referrals to other departments or outside agencies for resident care.
