Job summary

Signature HealthCARE in Greeneville, TN is seeking an LPN/LVN to deliver direct nursing care, administer medications, and supervise a nursing team on each shift. You will develop care plans, document accurately in the EHR, and contribute to a patient-centered care environment.

Able to work with a multidisciplinary team, maintain documentation, and uphold care standards. Prior supervisory experience and a current LPN/LVN license are highly valued.

Qualifications

• Current, active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one (1) year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Familiarity with electronic health records (EHR) systems.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety.
• Supervise and direct nursing assistants and medication aides during the assigned shift.
• Monitor residents’ status and notify physician of any changes in condition.
• Develop and review individualized resident care plans with the interdisciplinary team.
• Complete accurate clinical documentation per policy.
• Coordinate admissions, transfers, and discharges.
