Job summary

Trezevant Manor is seeking a Weekend LPN Supervisor for the Assisted Living community, 7:00 a.m. to 7:00 p.m.

The role provides clinical leadership, supervises nursing staff, ensures quality resident care, and promotes effective communication among residents, families, physicians, and the team. You will oversee daily nursing operations, assist with admissions and transfers, monitor changes in condition, ensure safe medication administration, and support the Assisted Living Manager.

Qualifications

• Must hold current LPN license in Tennessee.
• Minimum of 2 years Assisted Living and supervisory experience.
• Excellent leadership, organizational, and communication skills; ability to manage multiple priorities and work under pressure.
• Must be computer efficient and able to work with MatrixCare; CPR certification is required.

Responsibilities

• Supervise day-to-day nursing operations of the Assisted Living community.
• Provide clinical oversight during assigned shifts while supporting the Assisted Living Manager.
• Ensure residents receive individualized care based on physicians’ orders and service plans.
• Monitor residents for changes in condition and coordinate with physicians and families.
• Assist with admissions, assessments, transfers, and discharges; may work as an LPN.
• Participate in developing, implementing, and revising resident service plans.
• Conduct routine resident rounds to ensure quality of care and identify improvements.
• Coach, mentor, and train nursing staff; oversee orientation and training of new employees.
• Ensure safe administration of medications per orders and state regulations; review medication records.
• Investigate medication errors and provide corrective education as needed.
• Build positive relationships with residents and families; communicate status changes to team.
• Promote resident dignity, independence, and quality of life.
• Review clinical documentation and incident reports; complete follow-up investigations.

This position supports the Assisted Living Manager by supervising nursing staff, ensuring quality resident care, monitoring regulatory compliance, and promoting effective communication among residents, families, physicians, and the interdisciplinary team. The Nurse Supervisor serves as a clinical resource and leader, ensuring continuity of care and adherence to organizations policies and procedures.**PRIMARY DUTIES & RESPONSIBILITIES*** Supervise the day-to-day nursing operations of the Assisted Living community.* Provide clinical oversight during assigned shifts while supporting the Assisted Living Manager.* Ensure residents receive individualized care based on physicians’ orders, service plans, and resident needs.* Monitor residents for changes in condition and ensure timely assessment, intervention, physician notification, and family communication.* Assist with admissions, assessments, transfers, and discharges; May have to work as an LPN.* Participate in the development, implementation, and revision of resident service plans.* Conduct routine resident rounds to ensure quality of care and identify opportunities for improvement.* Assist with supervising, coaching, and mentoring nursing staff; Assist with orientation and training of new employees.* Assist with ongoing education and competency assessments; Monitor staff performance and provide feedback, coaching, and corrective action when necessary; Promote teamwork, accountability, and a positive work environment.* Ensure that medications and treatments are administered safely and accurately according to physician orders and state regulations.* Review medication records for accuracy and completeness; Monitor medication storage, documentation, and controlled substance procedures.* Investigate and follow up on medication errors and provide corrective education as needed.* Build positive relationships with residents and their families; Address clinical concerns promptly and professionally.* Communicate resident status changes appropriately with families, physicians, and interdisciplinary team members.* Promote resident dignity, independence, and quality of life.* Review clinical documentation; review incident and accident reports and complete follow-up investigations.* Complete audits assigned by the Assisted Living Manager.* Report for work on time, as scheduled.* Be knowledgeable of Trezevant’s Mission, vision and values.* Other duties as assigned/ required.## Qualifications**Qualifications:**1. Must hold current Licensed Practical Nurse (LPN) license in the state of Tennessee or multi state license.2. Minimum of 2 years Assisted Living and supervisory experience preferred.3.
