Qualifications

• Licensed LPN in good standing with the New York State Licensing Authority.
• Previous experience in care for seniors preferred.
• Case management experience.
• Possess a valid driver's license.

Responsibilities

• Coordinate and manage residents' health needs with the primary physician, resident, other care providers and family members.
• Supervise intake of new medical orders as well as distribution and discontinuation of residents' medications.
• Accurate and appropriate charting in resident files.
• Implement and ensure continued compliance with policy and procedures.
• Oversee/direct duties of staff (PCA's, CNA's, Home Health Aides).
• Monitor care needs and changes in condition; report to Program Manager.
• Meet customer service standards in a friendly, helpful manner with resident satisfaction as a priority.
• Maintain knowledge of State Regulations (DOH).
• Assist with scheduling and care coordination as needed.
• Maintain First Aid/CPR Certification.
