Job summary

Southcoast Health in Fairhaven, MA is seeking a Licensed Practical Nurse (LPN) to join our primary care team on the day shift.

This 32-hour position offers a $4,000 sign-on bonus, a comprehensive benefits package, and opportunities for professional growth. You will administer medications, provide patient teaching, and support care coordination in a community-focused health system.

Qualifications

• Graduation from an accredited school of practical nursing (LPN) is required.
• EHR experience is highly desirable.
• One year or more experience in related setting preferred.
• Current MA/Rhode Island LPN licensure is required; BLS certification required.

Responsibilities

• Provide direct patient care including medication administration and teaching.
• Use an Electronic Health Record (EHR) to update or access patient information per policies.
• Collaborate with the care team to maintain professional nursing standards.

Hours: 32 hrs Shift: Day shift; 8:00am - 4:30pm Location: Primary Care – Fairhaven, MA

Responsibilities

Position reports to the Director, Manager or designee. Perform a variety of duties involving direct patient care, such as medication administration, patient and family teaching, and testing/procedures in accordance with organizational policies, procedures, and protocols and within the scope of practical nursing. Utilize an Electronic Health Record (EHR) and other related software to update or access patient information as appropriate. Has knowledge and skills necessary to provide care, which is appropriate to the age group of patients regularly served. Works cooperatively with ancillary nursing and other patient team personnel in maintaining standards for professional nursing practice.

Qualifications

• Graduation from an accredited school of practical nursing or equivalent is required.
• Experience working with Electronic Health Record (EHR) is highly desirable.
• One year or more experience in related setting preferred.
• Current LPN Licensure in the Commonwealth of Massachusetts and/or Rhode Island is required.
• Basic Life Support (BLS) certification is also required.
• Must be fully vaccinated against seasonal Influenza and the COVID 19 virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hired.
• Compensation: Pay rate will be determined based on level of experience.

Pay Range

USD $28.66 - USD $44.60 /Hr.
