Grandview Healthcare Center is seeking a Licensed Practical Nurse (LPN) to provide direct resident care under physician orders and nursing standards. The role emphasizes documentation, assessment, and coordination with the nursing team in a sub-acute setting.

Qualifications

• CT Licensed Practical Nurse with active CT license
• Minimum 1 year of skilled nursing experience preferred
• Current CPR certification
• Excellent clinical assessment and documentation skills
• Strong English communication, both written and spoken
• Professional demeanor and ability to work under pressure
• Team player with good customer service skills

Responsibilities

• Assess and document resident progress in the medical record
• Monitor turning and positioning of residents
• Obtain changes in physician orders as indicated by progress
• Communicate resident condition and progress to charge nurses and the Director of Nursing
• Maintain a clean, organized treatment cart and restock supplies
• Follow Infection Control policies and procedures
• Administer medications and treatments as ordered by the physician
• Document pressure sore status weekly or as changes occur
• Monitor inventory of medications and supplies
• Educate residents and families about skin care upon discharge
• Participate in Quality Improvement activities
