Job summary

Grandview Healthcare Center is actively hiring Licensed Practical Nurses (LPNs) to provide high-quality care for residents in a skilled nursing and rehabilitation setting. We offer a modern facility with supportive leadership and a positive work environment.

The LPN will perform nursing duties as prescribed, document progress, and collaborate with the care team to ensure comfort and safety of residents, including medication administration and wound care as needed.

Qualifications

• LPN license current in CT.
• At least 1 year Skilled Nursing experience preferred.
• CPR certification required.
• Excellent technical, assessment, and documentation skills.
• Effective verbal and written English communication skills.

Responsibilities

• Assess and document resident progress in the medical record.
• Monitor turning and positioning of residents.
• Obtain changes in the physician's orders as indicated by the resident's progress or lack of progress.
• Communicate pertinent information about the resident's condition and progress to charge nurses, supervisors, and the Director of Nursing, as indicated.
• Maintain a clean, orderly treatment cart and restock treatment supplies.
• Follow Infection Control policies and procedures.
• Administer medications and treatments accurately as ordered by the physician and according to all facility policies and procedures.
• Document pressure sore status weekly or more often as the condition changes.
• Assist in monitoring the inventory of medications, medical supplies and equipment to be sure adequate supplies of skin care products are on hand to meet the needs of residents.
• Participate in educational programs for residents and their family members relative to skin care when a resident is being discharged to home.
• Participate in Quality Improvement activities, as assigned.
